“Forget me Not’ Taco Salad Sale Alzheimer’s Association
Fundraiser
When: Friday, July 26th from 10 am to 1 pm
Where: Camden Healthcare and Rehabilitation
197 Hospital Drive
Camden, TN 38320
Walk-ins are welcome, or we can deliver to local businesses just
fill out the attached form with a time, names and what is
wanted on your salad and we will deliver to your business— You
can fax the completed form to 731-584-2753 by noon on
Thursday, July 25th and you are all set for your lunch
experience. Thank you in advance for your support of the
