There was a lot of encouragement from the CNA’s and therapy team. Darryl B. was great!
Francis Quackenbush
Everyone here has been very helpful. His care has been great. He looks and feels better than when he started.
Mr. Paul A. Success Story
Mr. A came to us after a stay in the hospital following a surgical repair of his right leg. He suffered general weakness and was non-weight bearing on his right lower extremity. He was unable to transfer without moderate assistance and unable to walk at all. Mr. A required moderate assistance to complete self-care tasks upon admission. On his discharge, Mr. A was able to walk utilizing a RW with SBA and complete all self-care tasks Independently. He stated, “I am ready to go home, I could probably let y’all get me stronger but I just want to be home.” Mr. A reached Highest practical level before requesting to return home.
The rehab and nursing teams were great! The CNAs show a lot of concern and care.
Ms. Petricia J. Success Story
Ms. J came to us following a surgery to her right leg. She lived with her husband in Benton County in their private residence. She came in unable to place any weight on her leg. Ms. J was unable to complete her self-care tasks without maximal assistance from caregiver staff. Ms. J was unable to transfer herself as well. Ms. J discharged from the facility with the ability to transfer while maintaining her weight-bearing status with her husband just standing by in case she needed him. She could complete all self-care tasks with setup of the task supplies only required. Ms. J reached Highest practical level before returning home with her husband.

The therapy department really stood out and they seem to know what they were doing. It was hard work, but it paid off! The CNAs were good to me as well. Everyone has been very good and I am going to miss everyone!
98% of the staff have gone very far over what I can imagine, helping in every way. My heartfelt thanks and gratitude to all. God Bless!
Ms. Melba P. Success Story
Ms. P came to us after a stay in the hospital following a STEMI. She suffered general weakness and required SPO2 on at all times upon admission. Ms. P could not walk or get up and down without moderate assistance. Ms. P was able to get herself ready and walk out of the facility with no use of oxygen on her day of discharge without using as assistive device! This is Ms. P’s second stay at our facility in the last few years and she states, “Y’all have all been so good to me”. Ms. P is excited to return home and oversee her boys’ while they run her business. She also can’t wait to get her hands on those grandkids. Good luck Ms. P!

Tammy, LPN, stood out to me. She seems like she cares about everyone and takes good care of her patients. There were a lot of precautions in place due to Covid-19, and I was nervous about going into the center, but the staff made me feel safe!

Thank you for the wonderful care of our father while he was in your facility. The staff were always warm & friendly, we also appreciate all their hard work. Mitch in Therapy was great! Dad started walking again with his walker.
Forever grateful.
